1. Tuition for all dance classes is divided into three payments for your convenience. You will receive a statement via email prior to your payment due date. Payments received after the due date (generally the 1st of the month) are subject to a late fee of $15 per month, which will be strictly enforced.

2. Preschool Classes are offered in two sessions. Full payment is due at the beginning of each session.

3. Refund Policy: The studio does not issue refunds to enrolled students for classes missed due to illness, vacation, religious observance, or for any other reason. Any missed classes can be made up during the season. Registration and enrollment represents a financial commitment throughout the end of the season (June). In the event of withdrawal from the program, no refunds are given.

4. In the event of a dispute for breech of contract, the contractual liability of Mariann's School of Dance shall not exceed the amount of one year's tuition.

5. I understand and acknowledge that dance and exercise classes involve physical movement and exertion. I represent that the registrant(s) named above are in good physical health and are capable of performing the exercises and dances required for participation in the classes in which registrant(s) are enrolled. The undersigned, and all heirs, executors and assigns, waive all claims against Mariann's School of Dance, its agents, servants and assigns, for personal injury sustained out of participation in any classes or other activities in connection with the enrollment in this school.

6. It is the responsibility of the parent/guardian to be aware of all studio activities such as performances, extra classes, and dates the studio is open or closed. The studio will post notices on the Information Board as well as email reminders to parents/guardians.

7. Consent. In the event of an emergency or injury, Mariann's School of Dance is authorized to notify me. If I cannot be reached in an emergency, I give permission to the staff of Mariann's School of Dance to render all necessary aid and/or to act in my behalf to obtain emergency medical treatment for the registrant(s) listed above for any illness or injury that may occur while attending the school.

8. Release of Liability. As the legal parent or guardian, I release and hold harmless Mariann's School of Dance, its owners, agents, servants and assigns from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control of Mariann's School of Dance, its owners and operators or in route to or from any of said premises.

9. I agree to follow and abide by the written Policies and Regulations of Mariann's School of Dance currently in effect that are posted at www.mariannsdance.com and in the waiting area at the school.

10. I acknowledge that the original of the agreement and enrollment form will be held on file in the office of Mariann's School of Dance, and that I may obtain a photocopy of it upon request.

11. iPhoto Release. Mariann's School of Dance is hereby granted permission to take photographs of the students to use in brochures, web sites, posters, advertisements, and other promotional materials the school creates. I understand the student's name may be used in conjunction with a picture for studio newsletters / programs only.

Checking this box on the order form verifies enrollment and acceptance by the registrant(s) of the conditions of enrollment, the policies and regulations and tuition information of Mariann's School of Dance.

 

Tuition Information

1. The first payment is due upon registration. There is a non-refundable registration fee of $35 per single student or $50 per family. 

2. Multiple class discounts apply to individual students only.

3. We accept Visa or MasterCard for payments over $25.00.

4. Payments received after the due date are subject to a late fee of $15 per month, which will be strictly enforced.

5. There is a $30 service charge for any returned checks.  Once a check has been returned for nonpayment, only cash payments will be accepted.

6. We do not accept post dated checks.

7. No personal checks will be accepted after May 15, 2012. Cash or credit card payments only.

8. We are now offering the ability to make automatic payments. Contact the studio to make arrangements.

9. Contact the studio for information regarding alternate payment plans.

10. Students are expected to enroll for the entire season. However, we will prorate for those who join us late in the season. Classes will be pro-rated after November 1st only.  

11. The studio reserves the right to suspend a student’s participation in classes if their account is in arrears.

12. All invoices will be sent to the account electronically through email.
